Healthy Minds at Home

This Connections Learning and Life Skills Development Program provides in-home living support and assistance for individuals to be able to live independently and maintain a tenancy. Provided in partnership in the Caboolture area with Coast2Bay Housing Group.

Healthy Minds in Action

This Learning and Life Skills Development Program provides support for individuals to access and engage in community-based activities and education. The program assists in developing skills and independence in a variety of life areas, including: self-help, social skills, employment, leisure and social interaction.

Healthy Minds Packages

Connections has the capacity to service self-directed funding packages, respite packages and brokerage, and/or sub-contracted packages funded through the government, other organisations or self-funded individuals or families.

Connections also provides education and training to the community, including:

Healthy Minds at Work – this is a training programs designed for staff and management to gain skills and knowledge in managing and supporting people with a mental illness in the workplace.

NDIS – Better Prepared – Training can be provided for staff, Management or Board members in understanding and pre planning for the NDIS.
